Position Overview:
We are seeking an experienced and motivated Advanced Practice Registered Nurse (APRN) with a strong background in Pulmonary and Acute Care to join our expanding healthcare team. This position is dedicated to providing high-quality patient care in skilled nursing and rehabilitation facilities. The primary focus will be managing patients with pulmonary conditions and acute health issues in post-acute settings.
Key Responsibilities:
Deliver direct patient care with a strong focus on pulmonary and acute conditions in skilled nursing and rehabilitation environments.
Perform comprehensive health assessments, physical exams, and develop individualized treatment plans.
Prescribe medications and therapies in compliance with clinical best practices and state regulations.
Manage acute changes in patient conditions to prevent hospital readmissions.
Coordinate interdisciplinary care with physicians, specialists, and facility staff.
Complete timely documentation and maintain accurate electronic medical records (EMR/EHR).
Conduct admission, discharge, and follow-up evaluations to support optimal patient outcomes.
Educate patients and families regarding care plans, chronic disease management, and preventive health practices.
Qualifications:
Active APRN license.
DEA registration preferred.
BLS and ACLS certifications.
Minimum of 3 years clinical nursing experience, with a strong preference for backgrounds in pulmonary or acute care settings.
Experience working independently in fast-paced environments.
Proficiency with electronic medical record systems (such as PointClickCare or MatrixCare).
Strong clinical assessment, diagnostic, and critical thinking skills.
Preferred Experience:
Direct experience managing pulmonary diseases such as COPD, pneumonia, respiratory failure, and other chronic respiratory conditions.
Acute care management experience within skilled nursing or rehabilitation settings.
Knowledge of post-acute care workflows and senior care best practices.
