Job Summary
The Social Care Network Navigator plays a critical role in New York’s Social Care Network (SCN), helping Medicaid members access funded social care services such as housing supports, nutrition services, transportation, interpersonal safety supports, and other Health‑Related Social Needs (HRSN) benefits.
Navigators ensure members move smoothly through the SCN workflow — from referral to eligibility confirmation, service authorization, provider connection, and follow‑up — while maintaining compliance with SCN standards, documentation requirements, and data reporting.
This role is ideal for someone who thrives in community‑based work, understands social determinants of health, and can coordinate across CBOs, health plans, and SCN hubs.
Key Responsibilities
- Member Navigation — Guide Medicaid members through SCN services, ensuring they understand available supports and how to access them.
- Eligibility Verification — Confirm member eligibility for SCN-funded services using SCN platform tools and CBO protocols.
- Service Coordination — Connect members to appropriate CBOs delivering housing, nutrition, transportation, or other HRSN services.
- Referral Management — Receive, triage, and process referrals from health plans, providers, and community partners.
- Care Plan Support — Assist members in developing individualized social care plans aligned with SCN service categories.
- Documentation & Reporting — Maintain accurate records in SCN-approved systems; ensure compliance with billing, encounter reporting, and quality metrics.
- Warm Handoffs — Facilitate smooth transitions between CBOs, health plans, and social service agencies.
- Follow-Up & Engagement — Conduct check-ins to ensure services were delivered, barriers resolved, and outcomes documented.
- Community Partnerships — Build relationships with local CBOs, housing agencies, food providers, transportation vendors, and safety organizations.
- Quality & Compliance — Adhere to SCN operational standards, privacy requirements, and OMH/DOH guidance.
Pay: $20.00 - $25.00 per hour
