This full-time position plays a vital role in supporting residents and their families by assisting with psychosocial needs, care planning, and discharge coordination.
Key Responsibilities:
Assist the Social Services Director with resident assessments, care plans, and documentation
Support residents and families with emotional, social, and adjustment needs
Participate in interdisciplinary care plan meetings
Assist with discharge planning and coordination of community resources
Advocate for residents’ rights and ensure compliance with state and federal regulations
Maintain accurate and timely documentation in accordance with facility policies
Qualifications:
Minimum 1 year of experience in social services within a skilled nursing facility (required)
Knowledge of long-term care regulations and resident rights
Strong communication, organizational, and interpersonal skills
Ability to work effectively with residents, families, and interdisciplinary teams
