GENERAL STATEMENT OF POSITION:

Under general supervision, performs and/or supervises delivery of health services for all residents; directs and manages care staff to ensure the safety and wellbeing of the residents.

REPORTS TO: Clinical Services Director

SCOPE:

Supervisory Responsibility: Supervise the resident care team of the community that includes the following positions:

  • Medication Tech/QMAPs
  • Resident care assistant


ESSENTIAL FUNCTIONS:

The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

  • Know and understand the vision, purpose, values and goals of Anthem Memory Care.
  • Will provide Medication Tech/Lead support in supervision and direction of Resident Care Assistants.
  • Will provide Clinical Services Director input and will follow/or appointed designee to follow the directives for medical care needs (medications/treatments)
  • Assist Clinical Services Director in providing orientation to new employees; including care skills demonstration and evaluate for skill competency prior to care staff giving care.
  • Assists Clinical Services Director with performance evaluations and disciplinary actions as needed.
  • Assist the Clinical Services Director in providing in-service training for resident assistants/Medication Tech on a routine basis and one-to-one training as needed
  • Creates and maintains resident care communication system to ensure quality care on each shift
  • Monitors resident rooms, dining room, living room, attendant station, bathing rooms and medication rooms ensuring they are clean and orderly
  • Monitors supplies on hand, including linens and towels and orders as necessary. Maintain records and Central supplies. Monitor incontinence program and orders as necessary.
  • Supervises resident meals and monitors for appropriate diets being followed
  • Performs duties as manager on duty on a rotating basis
  • Performs other related essential duties as required.
  • Responsible for working shifts as needed for Care Giving or Medication Tech as needed
  • Protect confidential information of team members, residents and vendors
  • Assist Staff Development Director with, interviewing applicants and scheduling for RCA and QMAPs.
  • Accepts call offs after hours and performs outreach tasks to obtain adequate staffing as per staffing requirements.
  • Assures that social model of assisted living is in operation;
  • Assists with family contact when medical issue is involved and provides information about residents' conditions per Clinical Services Directors supervision.
  • Provides input about residents condition to the Clinical Services Director;
  • Assists in development of annual strategic plan goals for the health issues in the resident services department; actively participates in the total strategic plan goals for the resident services department;
  • Responds to emergency situations;
  • Acts as liaison with doctors and residents and care staff;
  • Conducts work tasks safely and in compliance with the community safety program;
  • Provides effective and courteous service to all residents, guests and co-workers;
  • Assists in other departments as needed;
  • Schedules transportation for residents to appointments if resident is unable;
  • Performs other related essential duties as required.
  • Organizes and thins charts as directed by CSD
  • Serves as a Lead Support in supervision of Resident Care Assistants and Qmap
  • Data entry into Electronic Health System
  • Organizing and prepping resident chart in preparation for move in
  • Closing out chart and safe storage of charts upon move out


MINIMUM TRAINING AND EXPERIENCE:

A High School Diploma, Medication Tech or QMAP certification, vocational/technical training preferred; Have at least one years of Supervisory experience, preferably in a geriatric setting; or any equivalent combination of training and experience that provides the required skills, knowledge and abilities.

CERTIFICATION AND CLEARANCE REQUIREMENTS:

  • Requires First Aid and CPR Certification
  • Must receive criminal records clearance from the Department of Justice and FBI;
  • Must have health screening and T.B. / CXR clearance.


KNOWLEDGE AND ABILITIES REQUIRED TO PERFORM ESSENTIAL JOB FUNCTIONS:

Knowledge of:

  • Policies, procedures, methods, and practices of the community;
  • Medical terminology;
  • Methods and procedures involved in handling emergency situations;
  • Use of medical supplies and equipment;
  • Medication dispensed to residents;
  • Supervisory methods and techniques;
  • Records, graphs, reports, etc. used in the daily performance of duties;.


Physical Requirements:

  • Ability to operate automated office machines including computer, calculator, telephone, and copier;
  • Ability to operate and use a variety of health care equipment and tools including stethoscope, blood pressure cuff,oxygen, emergency and disaster equipment, etc.;
  • Ability to utilize a variety of clerical, secretarial, medical and computer supplies;
  • Must be physically able to assist residents who have fallen and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects;
  • Must be able to lift and/or carry weights of residents to position or transport them;
  • Ability to perceive and discriminate colors, sounds, textures, odors, and forms.
  • Responsible for making frequent care rounds to monitor good resident cares, provide assistance as needed with cares, and provide education to staff during cares. Motivation and coaching team members


Supervisory Responsibilities:

  • Ability to assign, review, plan, and coordinate the work of other employees
  • Ability to provide instruction to other employees


Mathematical Knowledge:

  • Ability to add, subtract, multiply, divide, calculate decimals and percentages
  • Knowledge of and ability to use the metric system for medications

  • Ability to apply principles of rational systems
  • Ability to use judgment and respond calmly in stressful situations
  • Ability to use deductive reasoning, applying general rules to specific problems to produce answers that make sense
  • Ability to use inductive reasoning, combining pieces of information to form general rules or conclusions (includes finding a relationship among seemingly unrelated events).
  • Ability to arrange things or actions in a certain order or pattern according to a specific rule or set of rules (e.g., patterns of numbers, letters, words, pictures, mathematical operations, observations of human behavior)

Language and Interpersonal Communication:

  • Ability to comprehend and correctly use a variety of informational documents including resident records, telephone calls, lab reports, x-rays, attendance records, letters, time sheets, resident reviews, job applications and budgets
  • Ability to comprehend a variety of reference books and manuals including PDR, medical books, reports, journals, resident nursing notes, payroll summary and resident review
  • Ability to prepare performance appraisals, weekly reports, resident reviews and other job related documents using prescribed format and conforming to all rules of punctuation, grammar, diction and style
  • Ability to record and deliver information, to explain procedures, maintain confidentiality of restricted information, and to follow instructions
  • Ability to use and interpret medical, counseling, marketing, and personnel terminology and language
  • Ability to communicate effectively with residents and families, clients, physicians, nurses, staff, resident assistants, supervisor and client representatives verbally and in writing
