Responsibilities:
- Coordinate with interdisciplinary teams including nursing, therapy, and medical staff to ensure seamless service delivery
- Manage medication ordering process, catheter care, and other processes following HIPAA regulations and safety protocols
- Supervise nursing staff and care aides to uphold high standards of patient care and compliance with healthcare regulations
- Facilitate family communication, and documentation in electronic health record (EHR) systems such as PCC
- Conduct incident investigations related to health issues or any other concerns while ensuring adherence to infection control standards
- Work closely with HWD to ensure care plans and tasks are followed
- Ability to communicate effectively with healthcare providers regarding resident care concerns
- Be available to work various shifts based on the needs of the community
- On call as necessary for shift coverage and weekend rotation with HWD
Skills
- Extensive experience with EMR/EHR systems including PCC for accurate medical documentation
- Strong knowledge of medical terminology, and utilization review processes
- Proven leadership skills with supervising experience in long-term care, assisted living, or hospital environments
- Skilled in patient care such as bathing, dressing, grooming and transfers
- Expertise in managing complex cases involving dementia care, behavioral health issues, or geriatrics
- Ability to work effectively with individuals and teams in a positive manner across diverse age groups and cultures
- Knowledge of infection control practices essential for maintaining a safe environment
