Nurse Practitioner Palliative and Primary

Job Overview
Anpex Health is seeking a dedicated and skilled Palliative and Primary Care Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience in various medical settings. This role involves providing comprehensive care to patients, including assessment, diagnosis, and treatment planning. The Nurse Practitioner will work collaboratively with physicians and other healthcare professionals to ensure optimal patient outcomes.

Duties

• Conduct thorough physical examinations and patient assessments to determine health status.
• Develop and implement individualized care plans based on patient needs.
• Perform diagnostic evaluations and interpret laboratory results.
• Provide education on health management, disease prevention, and wellness strategies.
• Manage acute and chronic conditions in geriatric population.
• Utilize EMR/EHR systems for accurate medical documentation and patient records management.
• Collaborate with multidisciplinary teams in a SNF and ALF setting.
• Participate in clinical research initiatives to enhance patient care practices.
• Ensure compliance with HIPAA regulations and NCQA standards.

Experience
Candidates should have a robust clinical background with experience in the following areas preferred:

• Medical office experience including triage and patient intake
• EMR systems
• Skilled Nursing Home experience preferred
• Emergency medicine or urgent care settings
• Familiarity with CPT coding, ICD-9/ICD-10 coding practices
• A commitment to ongoing professional development is essential for success in this role.
