Essential Functions
- Conduct comprehensive in-home patient assessments, including vital signs, medication reconciliation, and focused physical observations
- Assist with and facilitate Nurse Practitioner (NP)-led telehealth visits using mobile devices and telehealth platforms
- Communicate clinical findings clearly and accurately to the NP and interdisciplinary care team
- Complete timely and accurate documentation in the electronic medical record (EMR)
- Identify and report changes in patient condition or safety concerns
- Provide patient education and reinforce care plans as directed by the NP
- Support care coordination and continuity between in-home visits and virtual care
- Ensure compliance with clinical protocols, company policies, and HIPAA regulations
- Active Licensed Practical Nurse (LPN) license in good standing
- Experience in home care, geriatrics, or primary care preferred, but not required
- Training provided for telehealth workflows and in-home assessment model

Salary : $55,000 - $62,000
