Job Description
Nature of Work:
As part of a member of a two-person team, support the county’s Hub and Spoke program. The program is designed not only to further expand access to buprenorphine to those with opioid use disorders residing in the county, but will also provide the needed coordination to create a robust bidirectional transfer of individuals based on level of need. The team works with the spoke sites to coordinate patient care within the hub-and-spoke network. The team travels to multiple spoke sites, meeting regularly with physicians and other staff to discuss cases; provide assessments, recommendations, and referrals; develop protocols; and coordinate among staff and sites providing services for the individual.
Education
Minimum Qualifications
Associate degree in counseling or human services or related field from an accredited college or university.
Experience
Two years of experience in substance use treatment. Ability to conduct screenings and assessments for substance use disorder treatment with an understanding of ASAM levels of care and DSM-5 diagnostic criteria for opioid use disorder. Experience providing care coordination and case management services.
License Or Certification
LMSW, LCSW-C, LGPC, LCPC, LCADC, CAC-AD, CSC-AD, ADT
Notes
Must have a personal vehicle available for travel to perform work duties and a current Maryland driver’s license with a clear driving record.
Desired Or Preferred Qualifications
Good standing as a Licensed Master's Social Worker (LMSW), Licensed Graduate Counselor (LGPC), Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Alcohol and Drug Counselor (LCADC) or Licensed Clinical Social Worker–Certified (LCSW-C). Certification by the Board of Professional Counselors as a CAC-AD, CSC-AD and ADT will be considered.
