Job Type: Full-Time (80 hours per pay period)
Schedule: Primarily Monday–Friday, day shift

Pay: $41.28 – $46.76/hour (based on experience)

Your Role as an RN Case Manager:
As an RN Case Manager, you will oversee and coordinate all aspects of resident care while supporting nursing staff across the campus. You’ll play a key leadership role in ensuring quality care, regulatory compliance, and strong clinical outcomes.
Key Responsibilities:
- Coordinate and oversee resident care using the nursing process
- Supervise and support nursing staff (RNs, LPNs, TMAs, NARs)
- Perform resident assessments and respond to changes in condition
- Develop, update, and monitor care plans and clinical documentation
- Ensure compliance with state and federal regulations
- Participate in care conferences, audits, and quality improvement initiatives
- Collaborate with interdisciplinary teams, providers, and families
- Assist with admissions, resident evaluations, and care coordination
- Provide education, coaching, and support to nursing staff
- Step into direct care or emergency situations as needed
What You Bring:
Required:
- Active Minnesota Registered Nurse (RN) license
- Minimum 2 years of long-term care or related clinical experience
- Strong clinical assessment and decision-making skills
- Ability to work independently and manage multiple priorities
- Excellent communication and leadership abilities
Preferred:
- Experience with MDS/RAI processes
- Knowledge of Medicare guidelines
- Previous leadership or supervisory experience
