Areas of Responsibility:

Individuals

  1. Participate in the intake and assessment of families/ children appropriate for the program
  2. Maintain regular contact with family/caregivers concerning the participant and the care they receive at home at the center.
  3. Complete weekly in-home education sessions with assigned clients
  4. Facilitate bimonthly group sessions for assigned clients
  5. Manage a yearly caseload of up to 20 clients
  6. Implement the Nurturing Parenting Program in a way that is culturally competent and engaging to participants.
  7. Conduct a comprehensive psychosocial assessment (face to face) of new clients and annual reassessments of existing clients
  8.  Ensure referrals are submitted on a timely basis and ensure appropriate follow-up with successful completion.
  9. Create Service Plans and update as required
  10. Prepare participant discharge plans.
  11. Serve as a continuous point of contact for the client, family/caregiver(s) and other appropriate parties.
  12. Coordinate and facilitate family/caregiver(s) support sessions.
  13. Consult with other professionals involved in the treatment of the client in a designated caseload.
  14. Support, and educate parents of children with special needs and disseminate information on their rights and services.
  15. Provide case management services and assist families in assessing services.
  16. Assist families in negotiating and solving situations with educational or other service providers.
  17. Support parents during educational meetings and other necessary meetings for the improvement of the child’s well-being.
  18. Ensure all required trainings are completed by the required due dates.

 

Program

  1. Process and follow up on incoming calls, including those from prospective individuals and/or other community agencies and referrals.
  2. Maintain/submit accurate, timely documentation (intake, treatment plan, session notes, assessments, units of service, hours worked, outcome, and demographic information). Submit all reports (weekly, monthly, quarterly) by the deadline including Paycom.
  3. Maintain organized, complete individual’s charts and documentation
  4. Update individuals, outcomes, and resources databases on a regular basis.
  5. Conduct periodic chart reviews and follow up on any deficiencies.
  6. Assist with the monitoring process.

 

Community

  1. Disseminate program information to the community.
  2. Network with other agencies and attend community events.
  3. Maintain, update, and organize information about community resources appropriate for children with special health care and educational needs.
  4. Perform all other duties as assigned.
Qualifications:

MINIMUM EDUCATION AND WORK EXPERIENCE REQUIREMENTS:

  1. Bachelor's Degree in Social Sciences or a relevant field
  2. Minimum 2 years of case management experience.
  3. Computer literate (Microsoft Office and Google).
  4. Experience working with families and/or children with developmental disabilities and some familiarity with educational rights, special education programs, and services.
  5. Strong interpersonal skills, good communication skills (verbal and written), and time management skills.
  6. Bilingual, preferred.

 

WORKING CONDITIONS: Duties are in an office environment with moderate noise levels.

    1. Able to lift 50 lbs. in a safe and appropriate manner utilizing standard orientation techniques.

    3. Must be able to hear see, and communicate effectively in writing, using the English language, with or without reasonable accommodation.
    4. Ability to work cooperatively with all levels of staff, exercising effective problem-solving, reasoning and critical thinking skills.
    5. May be required to accomplish job duties using various types of equipment/supplies to include, but not limited to, pens, pencils, calculators, computer keyboards, telephone, etc.
    6. Moderately sedentary position requiring computer and telephone work.
