What are the responsibilities and job description for the ASDSP position at Ann Storck Center Inc?
Areas of Responsibility:
A.Ensures high quality care is provided to the Individuals.
1.Functions as Circle of Support member to promote optimal individual/staff interactions.
2.Uphold and protect individuals’ rights, cares for individuals’ personal property, maintains confidentiality, offers choices, etc.
3.Feeds, bathes, and assists with self-care and grooming.
4.Ensures DSPs provide the appropriate level of assistance to allow the individuals maximum independence during daily living routines.
5.Ensures that meals provided according to each individual’s diet, preference and restrictions.
6.Reports medical concerns to the Nurse immediately.
7.Responds to the needs and preferences of the individuals’ noting requests or reactions as they communicate them.
8.Transports or arranges transportation for individuals to appointments, school and day programs, outings and activities away from the facility.
9.Participates in recreational activities and outings with the individuals.
10.Conducts inventories of each individual’s clothing, hygiene and supplies; Submits replenishment list and completes shopping as needed; Enters new personal possessions into inventory log and removes damaged or discarded items from inventory.
B.
Ensures that active treatment plans, programs and activity guides are followed properly.
1.Ensures that physical, nutritional management, meal preparation, menu and behavioral and physical management guidelines are followed.
2.Completes physical management, nutritional management, mealtime monitoring and behavioral management monitoring as scheduled.
3.Ensures that in-house group and individual leisure time activities are implemented; ensures individuals participate in special recreational activities.
4.Provides training to employees on active treatment plans using approved training methods; Monitors staff and provides training and assists with retraining and problem solving as needed.
5.Assists in completing skill maintenance checkoffs monthly for each individual.
6.Ensures that reports, data sheets, logbooks, etc. are completed accurately and in a timely manner.
C.
Fosters team spirit to ensure coordination of efforts.
1.Maintains a positive work attitude and effective and open communications with co-workers and supervisors; shows initiative to solve problems as they arise.
2.Participates in various meetings as appropriate (e.g., Circle of Support, Quarterly Reviews, Proposal,Individual, Behavior Management, etc.).
3.Represents the ASC professionally with individuals from other agencies, medical professionals, guardians, families, friends of individuals, etc.
D.
Ensures that the house operates in an orderly fashion.
1.Monitors the condition of bathrooms, sinks, tubs, showers, floors, positioning equipment, beds, etc., and reports safety and/or health hazards to the Residential Manager; submits work orders for repairs and maintenance and follows through to ensure completion.
2.Completes periodic walk-throughs to keep house in proper condition.
3.Ensures that Incident Reports are accurately completed and submitted in a timely manner; participates in investigations and preparing plans of correction for incident reports and surveys.
4.Participates in disaster drills, emergency drills and safety inspections.
5.Performs supervisor duties as directed and/or in absence of SDSP and/or Manager.
6.Provides OJT to new hires and in-services employees as needed.
7.Assists in making daily assignments (including breaks and lunch periods).
8.Ensures that employees properly complete assignments/job responsibilities in a timely manner.
9.Ensures that staff follows Daily Activity Guides, recreation and house duty schedules.
10.Ensures that all documentation is completed correctly and in a timely manner.
11.Monitors and assists in the evaluation of employee performance. Consults with SDSP, Group Home Manager regarding performance issues and counseling as directed and in compliance with federal, state and local laws and ASC policies and procedures.
12.Monitors attendance and resolves shift shortages; monitors overtime of staff, keeping it at a minimum.
13.Keeps supervisor informed of unusual incidents or problems which require intervention and/or follow-up.
E.

MINIMUM EDUCATION AND WORK EXPERIENCE REQUIREMENTS:
1.High School Diploma or GED certification.
2.At least two years of verifiable experience as a home health aide, caregiver, nursing assistant or other care professional; experience working with challenging behaviors, preferred.
3.May be required to have a Florida driver’s license and good driving record.
4.Effective problem-solving, reasoning and critical thinking skills.
5.Ability to maintain confidentiality when handling sensitive data.
6.Computer literate (Microsoft Office).
WORKING CONDITIONS: Primary duties are in an ICF environment with moderate noise levels. First, second or third shift; Weekends, holidays and overtime may be required. May be required to operate ASC vehicles to transport individuals. Position is categorized as essential personnel for the purposes of providing disaster services.
