We are currently seeking a Licensed Clinician to provide comprehensive mental health assessments, diagnoses, therapy, and treatment planning for clients. This role offers the opportunity to collaborate with a multidisciplinary team to ensure each client receives the care and support they need.
Key Responsibilities:
- Conduct assessments and diagnostic evaluations across all age groups.
- Develop individualized, therapy-focused treatment plans within the first 30 days of service.
- Provide ongoing individual therapy with documented progress and quarterly reviews.
- Offer individual and group supervision to care coordinators and other field personnel as needed.
- Monitor and justify continued services through thorough chart reviews.
- Develop and implement crisis plans when appropriate.
- Collaborate with medication providers and care coordinators to coordinate care and track progress.
- Maintain clear and timely documentation for all client interactions on a weekly basis.
- Facilitate training to staff to enhance skills provision three times a year
Qualifications:
- Master’s degree in Social Work, Counseling, Psychology, or related field.
- Active clinical license (LMSW-cc, LCPC-c, LCSW, LCPC)
- Strong clinical assessment and diagnostic skills.
- Experience in individual therapy and treatment planning.
- Ability to work collaboratively within a multidisciplinary team.
- Excellent organizational and documentation skills.
