As a Care Coordinator, you’ll be the first point of contact for new clients and families. You’ll guide them through the intake process, match them with the right caregiver, and stay in touch to make sure their care experience runs smoothly. This role is ideal for someone who’s organized, personable, tech-savvy, and ready to make a real impact every day.
What You’ll Do:
- Manage all client intakes, including gathering service details, completing documentation, and coordinating assessments.
- Match caregivers to new clients based on compatibility, skills, and scheduling needs.
- Serve as the main point of contact for clients during their onboarding and throughout the early stages of service.
- Build and maintain strong relationships with key referral partners, such as independent and assisted living facilities, hospitals, and community organizations. Occasionally visit these partners in person to strengthen trust and ensure seamless coordination of care.
- Collaborate with the scheduling team to ensure a smooth handoff for ongoing care and any future changes.
- Maintain accurate and timely information in agency systems, including WellSky.
- Communicate with caregivers, families, and payer sources in a professional, compassionate manner.
- Participate in the on-call rotation to handle after-hours client or caregiver needs.
- Support the Office Manager and leadership team with client updates and coordination needs.
What You’ll Bring:
- Associate degree or higher in healthcare, social services, or a related field (Bachelor’s preferred)
- At least 2 years of experience in care coordination, healthcare scheduling, or a similar role
- Strong computer skills and comfort navigating multiple systems
- Excellent communication, organization, and follow-up skills
- A calm, compassionate approach when working with clients and families
- Familiarity with payer sources such as Private Pay, VA, IDOA, MCO, and DRS is a plus
- Prior homecare experience preferred
