Job Summary:

The primary function of the Scheduler is to process all aspects of scheduling nursing visits via Home Care Home Base software system.

Functions:

• Schedule and appropriately document patient schedules in a timely manner.
• Participate in coordinating care with management and patient interdisciplinary team.
• Provide effective communication to patients, staff members, other health care professionals and referral sources.
• Respond to agency and patient needs in a professional and creative manner.
• Demonstrate commitment, professional growth, and competency.
• Promote Agency philosophy and administrative policies to ensure quality of care.

Skills & Experience:

• Minimum 1 year of home health experience.
• Excellent computer skills required.
• Excellent interpersonal and organizational skills.
• Basic knowledge of scanner/fax/copier equipment.
• Phone etiquette skills required.
• Ability to take direction with a multitude of tasks.
• Typing ability 40-50 WPM.
• Knowledge of Medicare regulatory guidelines.
• A reliable source of transportation
• Valid/current auto liability insurance is required.
