What are the responsibilities and job description for the RN Case Manager position at ANGELA HOSPICE HOME CARE INC?
- Develop an accurate comprehensive care plan based on the patient assessment and revise as necessary.
- Document assessment, care plans and care provided according to Angela Hospice Home Care, Inc. policies and procedures.
- Report changes in patient condition to responsible party & take follow-up action as necessary.
- Perform basic nursing skills in a knowledgeable and competent manner that provide for continuity of care.
- Administer and document medications/treatments as ordered by the physician and according to facility policy and procedures.
- Determine scope and frequency of services needed based on acuity and patient/family needs.
- Instruct patients/families regarding medications, treatments, procedures.
- Assess ability of patient caregiver to meet patient needs.
- Communicate patient/family concerns and initiate corrective action.
- Accept on-call as assigned and necessary.
- Make periodic phone calls to referring physician as patient’s condition changes
- Minimum one year experience as a Registered Nurse, Strong Hospice/Homecare background preferred.
- Current certification in the State of Michigan required
- Hospice experience preferred
Salary : $65,000 - $72,000
