position summary
The Home Care Admit Registered Nurse (RN) is a member of the interdisciplinary team who provides hospice/skilled professional nursing care as prescribed by the physician and in compliance with the medical plan of care. They are primarily responsible for admitting the patient onto Angela Hospice services. The Home Care Admit RN is committed to the philosophy, goals, and objectives of Angela Hospice and the Felician Franciscan Core Values.

essential job functions
Provides educational information to patients and families regarding the hospice philosophy, eligibility requirements, admission procedures, and Angela Hospice Home Care Inc.
Completes an initial assessment of patient and family to determine hospice needs
Completes all the initial necessary admission paperwork with the patient and/or family member/caregiver to get the patient signed onto Angela Hospice services
Provides a complete physical assessment and history of current and previous illness(es)
Initiates the patient’s plan of care and includes the patient and the family in the planning process
Provides quality, professional nursing care to patients utilizing specialized nursing skills related to end-of-life care
Performs comprehensive assessment of assigned patients in a timely manner upon admission and as required
Assesses and evaluates patients and makes necessary revisions to the plan of care as patient status and needs change
Initiates appropriate preventive and rehabilitative nursing procedures
Administers and documents medications and treatments as prescribed by the physician
Orders medications as needed from pharmacy
Educates the patients and families regarding medications, treatments and procedures, disease process, and end of life care/process
Uses established confidential communication channels to communicate pertinent information
Communicates patient/family concerns to appropriate interdisciplinary team member and initiatives a corrective action if required
Assists the patient with activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care
Lifts, moves, and transports patients using proper body mechanics or lifting devices
Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given
Records pain/symptom management changes/outcomes as appropriate
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition
Communicates and collaborates with community health related persons to coordinate the care plan
Teaches the patient and family/caregiver self-care techniques as appropriate
Provides and maintains a safe environment for the patient
Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver
Ensures that arrangements for equipment and other necessary items and services are available
Maintains the comfort, privacy, and dignity of each patient and interacts with patients in a manner of compassion and empathy
Reports an incident or evidence of abuse or violation of residents rights to the Director of Patient Access, or any administrative staff
Provides clinical supervision of LPNs, SNAs, and hospice aides and delegates responsibilities when required

OTHER DUTIES
Maintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services
Ensures compliance with the Medicare conditions of participation and other state regulations that govern the provision of healthcare
Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies
Participates in organizational monitoring of the quality of medical services and quality improvement initiatives
Develops, maintains and upgrades professional knowledge and practice skills through attendance at seminars, conferences and participation in continuing education and in-service classes
Fulfills the obligation of requested and/or accepted assignments
Demonstrates knowledge in counseling patient/family in dealing with end-of-life issues
Supports department operations as directed as well as being assigned to any special projects
Performs tasks which are supportive in nature to the essential functions of the position, but which may be altered based upon the need of the organization
Meets on-going requirements to remain compliant with Federal/State and/or other regulatory guidelines
Participates in efforts and initiatives to promote growth and longevity of the organization within your departmental scope
Performs other duties as directed by the President/CEO, Patient Support Center & Telehealth Clinical Manager or other assigned designee

supervisory responsibility
This position does not have any direct supervisory responsibilities, but provides clinical supervision to LPS, SNAs and hospice aides. This position is required to orient employees as needed and will act as a mentor to staff and volunteers.
PHYSICAL REQUIREMENTS
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. The position requires sitting and standing for long periods. The position is also exposed to infectious diseases and requires lifting of a minimum of 50 pounds regularly. It also requires bending, stooping, turning and twisting on a regular basis. They require the ability to operate a computer, phone and other office equipment as needed. Manual dexterity is also required to perform tasks including computer tasks.
work environment
This position will be required to operate in an infield home health care setting under all types of weather conditions as required. The role requires working directly with patients and other team members.
expected hours of work
This is a position with a hospice organization open 24 hours a day, 365 days a year. Scheduled regular, additional and holiday shifts as well as work during inclement weather will be required.

This position includes travel routinely.
Qualifications:
Qualifications
Graduate of an accredited school of nursing
Current RN license in the State of Michigan
CPR Certified
Hospice experience in an inpatient or home care setting preferred
Desire and ability to care for geriatric, convalescent, chronically, seriously or terminally ill persons
Excellent organization and time management skills
Proficient computer knowledge and skill
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
Ability to work with little or no supervision
Capacity to lift, position and/or transfer patients
Auditory and visual skills
Proficient interpersonal relations and communicative skills
Knowledge of Federal/State, NHPCO, and CHAP regulations and guidelines
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements
