What You’ll Do
As a Nurse Clinical Consultant, you will serve as a trusted clinical partner to assigned facilities, supporting high-quality patient care while strengthening Andwell Health Partners’ relationships across the continuum of care.
- Build and maintain collaborative relationships with facility leadership and care teams
- Conduct regular on-site visits to identify patients who may benefit from home health, hospice, or related services
- Perform clinical reviews and skilled nursing assessments to support timely, appropriate referrals
- Provide education to facility staff on eligibility, levels of care, and best practices
- Participate in patient and family conversations regarding care options and goals
- Collaborate with physicians, case managers, intake, and interdisciplinary teams to support smooth care transitions
- Monitor patients with anticipated decline and support proactive planning
- Contribute to program growth by identifying unmet clinical needs and strengthening facility partnerships
What You’ll Bring
You bring strong clinical judgment, effective communication skills, and a relationship-focused approach to care delivery. You are comfortable working independently while collaborating across teams to support patient outcomes and organizational growth.
- RN/LPN clinical experience in home health, hospice, or related care settings
- Ability to assess clinical needs and communicate recommendations clearly and professionally
- Comfort engaging in consultative conversations with facility staff, patients, and families
- Strong organizational and time-management skills
- Knowledge of hospice philosophy, eligibility, and care transitions
- A collaborative, solution-oriented mindset aligned with Andwell’s mission and values
- Experience in relationship management, account support, outreach, or sales is a plus, particularly in healthcare or service-based environments
- Current R.N. or LPN license in the State of Maine.
- ANA certification in Home Health or Community Health Nursing is encouraged.
- Current CPR card required.
- One year of recent clinical nursing experience is required.
- A reliable car with insurance.
