Overview
We are seeking compassionate and dedicated Home Health Aides to provide high-quality care to clients in their homes and community settings. The ideal candidate will have a strong background in patient care, excellent observational skills, and the ability to assist with a variety of daily living activities. Duties
- Provide personal care including assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and document patient condition by observing physical and emotional status, vital signs, and changes in health
- Assist with medication administration following prescribed protocols and maintaining accurate medical records
- Support patients with memory care needs related to Alzheimer's disease and dementia, ensuring a safe environment
- Operate Hoyer lifts and other mobility aids to assist with transfers safely
- Perform catheter care, wound care, and other specialized medical tasks as trained
- Assist with meal preparation and feeding, ensuring nutritional needs are met
- Conduct light housekeeping tasks such as laundry and cleaning to maintain a safe living environment
- Document patient interactions accurately using EMR/EHR systems while adhering to HIPAA guidelines
- Communicate effectively with family members, healthcare providers, and team members regarding patient status and care plans
- Provide hospice or palliative care support as needed
Skills
- Proven experience in home health or long-term care environments; experience in assisted living or nursing homes is preferred
- Strong knowledge of medical terminology, medication administration, and medical documentation processes
- Ability to work independently while following detailed care plans and protocols
- Excellent patient observation skills for identifying changes in condition or behavior
- Compassionate approach to caring for individuals with developmental disabilities, Alzheimer's/dementia, or other cognitive impairments
- Proficiency in using EMR/EHR systems for documentation purposes
- Knowledge of HIPAA regulations to ensure patient confidentiality at all times
- Ability to perform CPR and handle emergency situations calmly and effectively
- Physical ability to assist with transfers using Hoyer lifts or other mobility aids; capable of lifting up to 50 pounds if required
- Flexibility to work various shifts including live-in employment options; reliable transportation is preferred for home visits
