Job Summary
We are seeking compassionate and dedicated Home Health Aides (HHAs) to provide exceptional in-home care to clients with diverse needs. The ideal candidate will be responsible for assisting patients with daily activities, ensuring their safety, and supporting their overall well-being. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring home and community care services, including those with developmental disabilities, dementia, or recovering from illness. Experience in long-term care, assisted living, or nursing homes is highly valued. The position requires adherence to HIPAA regulations and maintaining accurate medical documentation using EMR/EHR systems.
Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting
- Monitor and observe patients’ conditions, reporting any changes to healthcare professionals
- Administer medications as prescribed and document medication administration accurately
- Take vital signs including blood pressure, pulse, temperature, and respiration rates
- Support patients with mobility using Hoyer lifts or other assistive devices while ensuring safety
- Provide companionship and emotional support to promote mental well-being
- Assist with meal preparation and feeding according to dietary plans
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment
- Document all patient care activities in medical records or electronic health records (EHR/EHR systems)
- Follow infection control protocols including proper use of PPE and hygiene practices
- Support patients with specialized needs such as Alzheimer’s care, dementia care, hospice care, or memory care
Qualifications
- Prior experience in home health, nursing homes, assisted living, or long-term care preferred
- Knowledge of medical terminology and caregiving best practices
- Certification in CPR is required; additional certifications such as catheter care or hospice care are a plus
- Ability to work independently while following care plans and instructions from healthcare professionals
- Strong communication skills and compassion for working with seniors and individuals with disabilities or developmental challenges
- Ability to work within HIPAA guidelines to ensure patient confidentiality
- Experience working with EMR/EHR systems for documentation purposes is preferred
- Valid driver’s license may be required for travel between client locations
