Job Title: Registered Nurse Long Term Care (LTC)
Job Summary:
The Registered Nurse Long Term Care (LTC) will provide compassionate and high-quality care to residents in long-term care facilities. This individual will be responsible for assessing patient needs, developing care plans, administering medications, and coordinating with interdisciplinary teams to ensure the overall well-being of residents.
Responsibilities and Duties:
1. Conduct assessments of residents' health status and develop individualized care plans
2. Administer medications and treatments as prescribed by healthcare providers
3. Monitor and evaluate residents' progress and adjust care plans as needed
4. Collaborate with physicians, therapists, social workers, and other healthcare professionals to provide comprehensive care
5. Educate residents and their families on health management and disease prevention
6. Document all patient interactions, treatments, and progress notes accurately and in a timely manner
7. Provide emotional support and advocacy for residents in their care
8. Ensure compliance with all state and federal regulations pertaining to long-term care facilities
Qualifications and Skills:
1. Valid RN license in the state of practice
2. Bachelor's degree in Nursing (BSN) preferred
3. Previous experience in long-term care or geriatric nursing
4. Strong clinical assessment and critical thinking skills
5. Excellent communication and interpersonal abilities
6. Ability to work collaboratively in a multidisciplinary team
7. Proficiency in electronic medical records (EMR) systems
8. Dedication to providing compassionate and patient-centered care
