Job Title: Long Term Care (LTC) LPN
Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Long Term Care (LTC) facility. The ideal candidate will provide high-quality nursing care to residents in a long-term care setting under the supervision of Registered Nurses (RNs) and physicians.
Responsibilities and Duties:
1. Administer medications and treatments as prescribed by healthcare providers.
2. Monitor residents' health conditions and report any changes to the RNs and physicians.
3. Assist with daily living activities such as bathing, dressing, and feeding.
4. Collaborate with the interdisciplinary healthcare team to create and implement care plans for residents.
5. Educate residents and their families on health maintenance and disease prevention.
6. Maintain accurate and up-to-date medical records.
Qualifications and Skills:
1. Must be a graduate of an accredited LPN program and hold a current LPN license in the state of practice.
2. Prior experience in long-term care or geriatric nursing is preferred.
3. Strong communication and interpersonal skills.
4. Ability to work effectively in a fast-paced environment and prioritize tasks.
5. Compassionate and empathetic towards residents and their families.
