Job Title: PT: Home Health
Location: West Plains, MO
Setting: Home Health
Pay Range: $2516.80 - $2642.64 weekly
Schedule: 8-Hour Days (additional shifts may be available)
Contract Length: 13 Weeks
Candidate Type: Local and traveler friendly – travel reimbursement available, Local candidates only – travel stipend not available

Requirements:

• 15 years of experience as a PT
• Must have at least 4 years of experience in Home Health
• Valid Physical Therapist license (Texas license held; Missouri licensure required for this role)
• Willingness to obtain Missouri licensure
• US-based practice experience with active license for over 12 years

Responsibilities:
As a healthcare professional in a home health setting, you will:

• Deliver one-on-one, high-touch care to patients in their homes
• Conduct skilled nursing visits and perform assessments
• Create and manage individualized care plans
• Collaborate with families and interdisciplinary providers to improve patient outcomes
