Location: The Villages, FL
Setting: Home Health
Pay Range: $2551.20-2678.76 weekly
Schedule: 8-Hour Days (no additional shifts specified)
Contract Length: 13 Weeks
Candidate Type: Local and traveler friendly - travel reimbursement available; Local candidates only – travel stipend not available
Requirements:
- 2 years of experience as a PT
- Qualified applicants MUST have at least 2 years of experience in Home Health
- Valid Physical Therapist license
- Be willing to obtain Florida state licensure

Responsibilities:
As a healthcare professional in a Home Health setting, you will:
- Deliver one-on-one, high-touch care to patients in their homes, including assessments and care plan management.
- Conduct skilled nursing visits and collaborate with families and interdisciplinary providers to improve patient outcomes.
- Create and manage individualized care plans tailored to each patient’s needs.
