We are looking for a skilled Care Coordination Pharmacist Assistant to join our team at AmPharm LLC. In this role, you will serve as a liaison between the input, dispensing, and inventory departments, ensuring the smooth and efficient delivery of pharmacy orders while maintaining high standards of quality assurance across all pharmacy procedures.
The ideal candidate will have a strong background in pharmacy operations, with a certification as a Pharmacy Technician (CPhT) and experience in care coordination. Excellent communication and organizational skills are also essential for success in this role.
- Main Responsibilities:
- Coordinate daily cycle fill orders for timely delivery to facilities
- Manage and resolve incoming requests from facilities concerning pharmacy orders
- Coordinate with nursing facilities regarding equipment availability and delivery
- Obtain medication prescriptions for scheduled medications
- Refer specific clinical questions to pharmacists
- Manage medical records for selected facilities, including monthly Recertifications and MAR's
