Responsibilities:
- Provide skilled physical therapy services in the home setting
- Complete patient evaluations and develop individualized treatment plans
- Deliver therapeutic exercises, gait training, balance training, and mobility support
- Educate patients and families on home exercise programs and safety
- Complete accurate and timely documentation in compliance with agency standards
- Communicate progress and updates with the clinical team as needed
- Active New Jersey Physical Therapist license (required)
- Strong communication and interpersonal skills
