Key Responsibilities
- Build and maintain strong professional relationships with DDD Support Coordinators throughout New Jersey
- Actively promote Ameriwell Care’s services and increase referral activity
- Coordinate and manage the intake process for individuals entering services
- Serve as a primary point of contact for families, support coordinators, and referral sources
- Track outreach efforts, referral pipelines, and census growth
- Represent Ameriwell Care at professional meetings, networking events, and community engagements
- Support smooth onboarding and service launch for new individuals
- Experience in NJ DDD, healthcare, or home care services, preferably with knowledge of the New Jersey Division of Developmental Disabilities system
- Strong communication and relationship-building skills
- Organized with strong attention to detail
- Comfortable working both in the office and in the field
- Reliable transportation for travel within New Jersey
