The Bilingual Case Manager plays a vital role in providing nursing oversight and support within the Americare Adult Foster Care Program. This position assists caregivers with all aspects of members’ medical and health-related needs, serving as an advocate with physicians and other healthcare providers to ensure high-quality care.

- Facilitating communication between members, families, and medical professionals
- Supporting caregivers in implementing care strategies and managing chronic conditions
Fluency in both English and Spanish is essential to effectively serve our diverse member population and to ensure clear, compassionate communication across all levels of care.
Responsibilities
Under the direction and supervision of the AFC Provider RN, the Case Manager will:
- Ensure compliance with all applicable Adult Foster Care (AFC) Medicaid regulations, including oversight of mandated training for Caregivers.
- Complete all nursing assessments and coordinate applicable clinical evaluations required during the intake and admission process for AFC services.
- Participate in the evaluation and support of Caregivers.
- Develop and maintain positive relationships with Members and Caregivers, while monitoring, assessing, and supervising placements in collaboration with the Social Worker/Case Manager.
- Serve as a liaison with external agencies providing services to Members, such as day programs.
- Notify the Member’s Primary Care Physician (PCP) of any changes in health status.
- Maintain accurate and up-to-date medical records and documentation, including PCP Order Forms, daily logs, vital signs (when appropriate), medication sheets, monthly nursing notes, and health records.
- Develop, complete, and maintain semi-annual health status reports, individualized Plans of Care, and annual assessment tools as required by MassHealth.
- Train and supervise Caregivers in all aspects of their responsibilities, including the medical and personal care needs of Members.
- Conduct orientations for new Caregivers prior to providing personal care services, and offer ongoing training on health, safety, and aging-related concerns.
- Oversee the program’s medication management policies and procedures.
- Monitor Members’ health status through regular contact with Caregivers and conduct home visits as follows:
Level I: Every other month (alternating with the Nurse’s visits)
Level II: Monthly or more frequently, as warranted by the Member’s condition
- Document a nursing progress note for each visit or encounter, or more frequently as needed.
- Oversee and support the grievance process for both Members and Caregivers.
- Plan and coordinate discharges from the Adult Foster Care Program, participating in discharge planning as needed.
- Provide crisis intervention, which may include 24-hour on-call responsibilities.
- Actively participate in supervision, team meetings, and multidisciplinary case discussions.
- Attend required training and maintain all current certifications and licenses.
- Uphold the principles of The Advocates Way, ensuring all Members are treated with dignity and respect in accordance with Advocates’ Human Rights Policy.
- Perform all duties in accordance with agency policies and procedures.
Qualifications
- With a Bachelor's Degree (Nursing degree, preferred)
- Minimum of 1 year of recent experience in direct care for the elderly or adults with disabilities.
- Bilingual in English and Spanish is required.
- Strongly prefer candidates with demonstrated experience serving culturally diverse populations.
- Excellent oral and written communication skills, with the ability to interact effectively with members, caregivers, healthcare providers, community partners, and staff.
- Must be able to perform all essential job duties satisfactorily.
- Demonstrated sensitivity to the needs of the individuals served.
- Proficiency with Google products and intermediate computer skills required.
- High energy level, strong interpersonal skills, and a collaborative team approach.
- Valid driver’s license and reliable, insured vehicle required. Must pass a Motor Vehicle Record check.
- Must successfully pass CORI, SORI, and OIG background checks.
- Must complete a pre-employment physical exam and have received a tuberculosis screening within the last 12 months.
