What are the responsibilities and job description for the AFC Case Manager position at All Care?
All Care Resources is looking for a full-time Case Manager to join our Adult Foster Care team. Candidate is responsible for conducting home visits in the community, travel required.
This is a very rewarding program that assists/supports family members and friends in caring for persons in their home setting versus an inpatient placement. The case manager position offers some flexibility in scheduling but requires a high level of organization.
Work Schedule: Monday-Friday, 9am-5pm
Work Location: On the road in assigned territory, including East Boston and surrounding communities
- conducting the initial and ongoing evaluations of the AFC qualified setting; 2. obtaining a social history and conducting a psychosocial evaluation
- provide oversight to AFC caregivers in conjunction with the registered nurse
- participating in the development, implementation, and ongoing review of the AFC plan of care
- conducting on-site visits with each member at the qualified setting in accordance with 130 CMR 408.415(C)(5)
- completing a care-management progress note corresponding with each on-site visit or encounter and upon significant change
- assisting with obtaining information and accessing other healthcare and community services
- participating in discharge and transition planning and implementation
The care manager must have
- Bachelor’s degree, a social worker license from the Massachusetts Board of Registration in Social Work, and at least two years of recent experience working with elders or adults with disabilities;
- or a bachelor’s degree and two years of clinical experience in the care of elders or people with disabilities.
Job Type: Full-time
Pay: $27.00 - $32.00 per hour
