Description
American Traveler is hiring a Home Health RN with Homecare Homebase (HCHB) experience and a minimum of 1 year of Med/Surg experience for a 13-week travel contract in the Lexington, KY area.
Details
- Home health care setting serving an adult-only patient population
- Case management of approximately 30 patients with a daily point expectation of 6.5 points
- Responsibilities include Start of Care (SOC), recertifications, and daily visits
- Uses Homecare Homebase (HCHB) EMR — experience is required
- Schedule is 5 x 8-hour shifts, Monday through Friday, with a weekend and holiday rotation
- No on-call requirements
Requirements
- Active KY RN license or Compact (NLC) license currently in hand — pending licenses are not accepted
- Active BLS certification required — pending certifications are not accepted
- Minimum 1 year of Med/Surg experience in a hospital setting required
- Home health experience required
- Homecare Homebase (HCHB) experience required
- Valid driver's license required
- 2 professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, both including dates of employment and eligibility for rehire
Additional Information
- Conduct home visits including SOC assessments, recertifications, and routine daily visits for an adult home health caseload
- A clinical manager or educator is available 24/7 for emergencies and during business hours (8am–5pm) for general support
- Orientation consists of approximately 2 weeks, including visits with a work coach (approximately 12 orientation hours total)
- Phone and tablet are provided for use in the field
- Scrubs are required; no specific color requirement
- No clinical involvement beyond direct patient care responsibilities
- Holiday rotation is required
