Description
American Traveler is seeking an experienced RN - Case Manager for a community hospice position requiring a minimum of 2 years of experience and an active CO or compact RN license.
Details
- Community hospice setting, providing care in patients' homes, nursing homes, group homes, and assisted living facilities
- Hospice nursing sub-modality with an assigned patient caseload
- Leads an interdisciplinary team in delivering patient-centered care
- Monday through Friday, 8:00 AM – 5:00 PM schedule
- Occasional weekend backup and on-call responsibilities required
- 13-week contract
Requirements
- Active CO RN license or compact RN license currently in hand
- Minimum 2 years of nursing experience required
- Strong clinical assessment and critical thinking skills
- Experience in symptom management and generalist knowledge across disease processes
Additional Information
- Serve as the primary RN managing an assigned caseload of hospice patients and their families
- Educate patients and caregivers on personal goals of care and build caregiver confidence in home care
- Provide compassionate, patient-centered care with a focus on symptom management and quality of life
