Description
American Traveler is hiring an experienced RN Case Manager specializing in Behavioral Health for a 13-week ambulatory care assignment in Juneau, AK, requiring an active Alaska RN license and case management experience within the last 2 years.
Details
- Ambulatory care/clinic setting specializing in Behavioral Health and Psychiatry
- Provides case management services for outpatient Behavioral Health/Psychiatry patients of all ages, including those with substance use disorders (SUD)
- Coordinates medical care and medication refills with physicians, pharmacies, and insurance providers
- Conducts nursing evaluations, injections, and lab collection as appropriate
- Schedule options include Mon–Fri 8am–5pm, Mon–Fri 10am–7pm, or Tues–Sat 8am–5pm
- 13-week contract with a 40-hour work week
Requirements
- Active, unrestricted RN license in the State of Alaska required at time of consideration
- Minimum 1 year of RN experience required; case management experience must be within the last 2 years
- Current BLS certification required
- Associate's degree or higher in Nursing from an accredited program
- Proof of flu vaccine required
- Fingerprint card submission required prior to start; start date is a minimum of 6 weeks from receipt of fingerprint cards
Additional Information
- Facilitates care coordination including referrals, follow-up care, and continuity of services for Behavioral Health patients
- Makes brief patient contacts to educate and reinforce clinician directives and identifies needs for further nursing or case management support
- Collaborates with medical and behavioral health clinicians as part of an interdisciplinary team
- Delivers culturally sensitive, evidence-based care aligned with individualized treatment plans
- Documents assessments, interventions, and outcomes in the electronic health record
- Floating to other departments or assignments may be required
- Candidates must reside at least 50 miles from the facility to be eligible; housing and travel are not provided by the facility
