Description
American Traveler is seeking a Physical Therapist with at least 1 year of recent home health experience and an active state PT license for a day shift assignment.
Details
- Home health and hospice care setting serving a 60-mile radius around Valley City, ND
- Typical caseload is 5-7 patients per day with cases including post-surgery and fall recovery
- Monday through Friday day shifts, 8:00am–5:00pm, no weekends or holidays required
- Uses HomeCare HomeBase EMR system
- Assignment is 13 weeks in duration
Requirements
- Active ND Physical Therapy License or compact licensure required if applicable
- Minimum 1 year of recent physical therapy experience in home health
- Current BLS certification required
- Valid driver’s license and auto insurance required
- Must possess strong PT skills appropriate for home health settings
- Experience using HomeCare HomeBase is preferred
- Two professional references required: one supervisor from the past 12 months and one peer or supervisor from the past 3 years verifying employment dates and rehire eligibility
- Submission requires copy of driver’s license
Additional Information
- Responsibilities include evaluating and treating home health and hospice patients with rehabilitation needs related to surgeries, falls, and related conditions
- Travel within a 60-mile radius of Valley City, ND, is required to visit patient homes
- Three to five shifts of department-specific orientation provided at start
- Scrub or business casual attire required per department guidelines
- Candidates must not have worked full time, part time, or PRN at any CommonSpirit, CHI, or Dignity facility in the past 12 months
- Time off requests must be submitted upfront; additional RTO is not approved after submission
- Candidates must live at least 50 miles from the facility to be eligible for travel rates
