American Traveler is seeking a licensed Physical Therapist with at least 1 year of home health experience for a 13-week travel contract serving patients across the Hawaiian Islands, including the Big Island, Kauai, Maui, and Oahu.
Details- Home health setting serving a predominantly geriatric patient population (kupuna) in their residences across Hawai'i Island
- Occasional floating to cover Kauai, Maui, and/or Oahu may be required; all float-related expenses are covered
- Patient specialties include orthopedics, neurology, geriatrics, cardiopulmonary conditions (COPD, CHF, CABG, MI, PVD), post-stroke rehabilitation, and cognitive/neurological disorders such as Parkinson's disease and dementia
- Procedures and skills include gait training, OASIS assessments, plan of care development, patient/family education, home exercise program development, and post-op rehabilitation (total hip/knee replacement, spinal surgery)
- Productivity requirement of 25 units per week
- 1:1 patient ratio
- Day shift schedule, 5x8 hours (8:00–16:30)
- HomeCare HomeBase (HCHB) used for documentation; experience preferred
- Minimum 1 year of home health experience required
- OASIS documentation experience of more than 3 months required
- Current BLS certification (AHA) required
- Active Hawaii (HI) PT license required; license required if applicable
- Strong skills in geriatric, neurologic, and orthopedic patient populations
- Proficiency with adaptive and assistive devices, including Hoyer lifts, gait belts, walkers, sliding boards, wheelchairs, lower extremity prosthetics, and supplemental oxygen
- Experience with OASIS documentation, plan of care development, reassessments, and discharge planning
- Ability to identify environmental barriers and conduct home safety assessments
- Preferred: wound care, lymphedema management, manual therapy certification, and experience with MS, muscular dystrophy, spinal cord injury, or TBI
- First-time travelers are welcome to apply
- Scrubs of any color and closed-toed shoes required
- Candidates should be prepared to provide total months of start-of-care and OASIS experience, as well as frequency of Medicare patient starts of care per week, for consideration
- Preferred skills include acute care exposure, lymphedema management, compression device use, NDT techniques, and experience with complex neurological conditions
