Description
American Traveler is hiring an LPN for a community-based Assertive Community Treatment (ACT) program, requiring experience working with individuals with mental illness in a community setting, with a Monday–Friday daytime schedule and no weekends, holidays, or on-call.
Details
- Community-based Assertive Community Treatment (ACT) program — no inpatient beds or on-site clinical setting
- Patient population includes individuals with chronic mental illness such as schizophrenia and bipolar disorder
- Caseload of approximately 6–8 clients per day, seen one-on-one in the community
- Services include administering injections, delivering medications, conducting home visits, and patient check-in calls in the community setting
- This position requires the clinician to drive their own personal vehicle; mileage is reimbursable at the IRS rate
- Charting is completed in Epic; each patient encounter requires a separate Epic note
- Schedule is Monday–Friday, 8:00 AM–4:30 PM (8-hour days); no weekends, holidays, or on-call
- No floating required
- 13-week contract
Requirements
- Current LPN/LVN license required if applicable
- Minimum 1 year of LPN experience required
- Experience working with individuals with mental illness in a community-based program is required
- Current BLS certification required
- Valid driver's license required; candidates must reside more than 50 miles from the facility to be eligible
- Epic experience preferred
Additional Information
- Primary responsibilities include administering injections, delivering medications to clients in the community, conducting home visits focused on medication adherence, making patient phone calls, and completing daily documentation in Epic
- Attendance at a morning meeting each weekday is expected
- Orientation consists of shadowing with the current ACT RN and is focused on the specific patient population
- Business casual attire or scrubs of any color are acceptable
- Candidates who have previously worked at any WVU Medicine location must have taken a 2-year break before being considered
- Candidates who have worked within the WVU system for a cumulative total of one year must also take a 2-year break before returning
- All candidates interview directly with the hiring manager
- 2 supervisor/peer references required for consideration, including 1 supervisor reference; if the candidate worked a permanent position in the last 12 months, one reference must be from that employer
- A copy of your driver's license is required for consideration
