Position Overview
We are seeking an experienced Nurse Practitioner with a strong background in skilled nursing facilities (SNFs) to help oversee and support clinical operations within our growing post-acute care organization. This role is ideal for someone who understands the day-to-day workflow of nursing homes, enjoys building processes, collaborating with interdisciplinary teams, and improving operational efficiency across multiple facilities.
The ideal candidate is highly organized, proactive, and comfortable working both independently and cross-functionally to ensure operational excellence and high-quality patient care.
Responsibilities
Support clinical and operational initiatives across multiple post-acute care facilities.
Collaborate with facility leadership, nursing staff, and healthcare providers to improve workflows and operational performance.
Assist in the development, implementation, and refinement of clinical and operational processes.
Monitor key operational metrics and identify opportunities for improvement.
Help coordinate transitions of care and ongoing patient management initiatives.
Provide clinical guidance and support to internal teams as needed.
Participate in training, education, and quality improvement efforts.
Ensure compliance with applicable regulatory and documentation standards.
Serve as a liaison between internal teams and facility partners to facilitate effective communication and problem-solving.
Qualifications
Active Nurse Practitioner license in good standing.
Minimum of 2 years of experience working within skilled nursing facilities or other post-acute care settings.
Strong understanding of SNF operations, interdisciplinary care, and regulatory requirements.
Excellent organizational, communication, and leadership skills.
Comfortable working in a fast-paced, growing organization.
Experience with EMRs and healthcare technology platforms preferred.
Ability to manage multiple priorities while maintaining attention to detail.
Preferred Qualifications
Experience supporting multi-site operations.
Background in quality improvement, care coordination, or operational leadership.
Familiarity with Medicare and post-acute care reimbursement models.
