Position Summary:
Responsible for ensuring delivery of high quality care practices in the treatment of symptoms and promoting the highest quality of life for homebound patients and their families
Principal Responsibilities:
In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $120,000 to USD $170,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits. Principal Responsibilities:
- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
- Assesses, diagnoses and develops appropriate plan of care to treat homebound patients
- Monitors the patient’s condition and responsiveness to the treatment provided
- Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations
- Participates in organizational wide QAPI program and attends agency sponsored in-service programs
- Adheres to agency infection control and safety policies
- Adheres to the organization’s policy in regards to absenteeism and appearance
- Master’s degree in Nursing required from an NLN accredited program
- Licensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority; maintain a CDS and DEA
- National certification as an Adult or Geriatric Nurse Practitioner
- 2 years of APN experience and/or significant experience as a registered nurse
- Excellent communication, organizational and interpersonal skills
- Strong computer skills
