What do we do?

Our goal at American Pain Consortium is to support our clinics that help patients seeking relief from pain, numbness, and tingling sensations that are often related to neuropathic pain or neuropathy, among many other forms of chronic pain. Even patients who have tried other pain treatments without success often benefit significantly from safe, effective treatment at American Pain Consortium. Our clinical multidisciplinary approach to care delivers the correct intervention to every patient, every time through various pain management procedures including injections, open surgical cases with general anesthesia, physical therapy, and psychological services. Our Providers focus on helping individuals improve functionality to restore a more meaningful lifestyle.

APC is seeking an individual who is eager to learn, develop, and grow in a role in the Pre-Service Department located in the Support Office in Carmel, IN.

Job Summary

The Chronic Care Management Specialist performs care management for patients enrolled in Chronic Care Management (CCM) services, specifically targeting patients with 2 or more chronic conditions for service coordination. The Chronic Care Management Specialist works in collaboration and continuous partnership with enrolled patients and their family/caregiver(s), our team of providers, and community resources in a collaborative approach to increase patients’ self-management ability and to improve overall patient health.

Essential Duties & Responsibilities (What You’ll Do)

• Responsible for registry of chronic care management (CCM) patients
• Validates enrollment of CCM patients
• Conducts minimum of one 20-minute telephone counseling and education session per month with each enrolled CCM patient
• Complies with documentation requirements of the Chronic Care Management program by carrying out the care plan with the patient, family/caregiver(s), and providers by recording session details in the EHR/CCM system
• Monitors adherence to care plans, evaluates effectiveness, monitors patient progress in a timely manner, and facilitates changes as needed
• Creates an ongoing process for patient and family/caregivers(s) to determine and request the level of care coordination support they desire
• Facilitates patient access to appropriate medical and specialty providers
• Coordinates transition of primary care to specialty care in an effort to improve overall patient health
• Works closely with in-office providers to manage the day-to-day calls involving symptom control, medication management, and provide patient and family education
• Educates patient and family/caregiver(s) about relevant community resources
• Assists with the identification of high-risk patients (the chronically ill and those with special health care needs) and assists in the enrollment of patients into the CCM program
• Coordinates continuity of patient care with internal providers and possibly with external healthcare organizations
• Supports patient self-management of disease and behavior modification interventions
• Provides patient health counseling, education, and instruction
• All other duties as assigned

Education and/or Experience Requirements

• High school diploma
• Minimum 24 months administrative experience in a medical facility, health insurance, or related area preferred
• Knowledge of function and relations within a healthcare clinic environment required
• Strong working knowledge of chronic disease states including chronic pain, kidney disease, diabetes mellitus, chronic obstructive pulmonary disease, and congestive heart failure and basic medical management of these states preferred
• Basic computer skills including previous work with an electronic health record (EHR) system
