Care Manager (RN/LPN)

Join Our Skilled Nursing Team!

We are seeking a compassionate, organized, and clinically strong Care Manager (RN/LPN) to join our team.

The Care Manager serves as a key connection between residents, families, nursing, providers, and the interdisciplinary care team. This role focuses on coordinating resident care, monitoring clinical needs, supporting care planning, and helping ensure each resident receives consistent, individualized, and well-coordinated services throughout their stay.

If you're an RN or LPN with skilled nursing or long-term care experience who enjoys building relationships, coordinating care, and making sure nothing falls through the cracks, we'd love to meet you!

Position Overview

As a Care Manager, you will help oversee and coordinate the care of an assigned group of residents from admission through discharge.

You'll work closely with nursing leadership, floor nurses, CNAs, physicians, therapy, social services, dietary, MDS, and other members of the interdisciplinary team to ensure resident needs are identified, communicated, and addressed.

This position is ideal for a nurse who is clinically knowledgeable, highly organized, proactive, and comfortable communicating with residents, families, providers, and healthcare professionals.

What You'll Do

  • Coordinate and monitor the overall care of assigned residents.

  • Serve as a clinical resource and point of communication for residents, families, providers, and members of the interdisciplinary team.

  • Review resident clinical information and monitor for changes in condition, new orders, treatments, medications, and other care needs.

  • Participate in resident care planning and interdisciplinary team meetings.

  • Help ensure individualized care plans accurately reflect resident needs, goals, preferences, and current clinical status.

  • Collaborate with the Nurse Assessment Coordinator/MDS team to ensure resident assessments and documentation accurately reflect the care being provided.

  • Monitor nursing documentation for accuracy, completeness, and appropriate follow-through.

  • Coordinate with physicians, nurse practitioners, therapy, pharmacy, social services, dietary, and other healthcare partners.

  • Assist with new admissions, readmissions, transfers, and transitions of care.

  • Review hospital and provider documentation following resident returns to the facility and help ensure new orders and recommendations are appropriately implemented.

  • Communicate meaningful changes in resident condition to appropriate providers, leadership, and family members.

  • Follow up on labs, diagnostic testing, consultations, appointments, and provider recommendations as appropriate.

  • Identify potential clinical concerns and collaborate with nursing leadership to address issues proactively.

  • Participate in quality improvement initiatives and efforts to reduce avoidable hospitalizations and readmissions.

  • Support infection prevention, fall reduction, skin integrity, medication management, and other clinical quality initiatives.

  • Assist with discharge planning to promote safe and successful transitions of care.

  • Advocate for residents while promoting dignity, choice, safety, and quality of life.

  • Maintain compliance with facility policies and applicable state and federal regulations.

What We're Looking For

Required

  • Current and unrestricted RN or LPN license.

  • Nursing experience in skilled nursing, long-term care, rehabilitation, or a similar healthcare environment.

  • Strong clinical assessment and critical-thinking skills.

  • Excellent communication and interpersonal skills.

  • Strong organizational and time-management abilities.

  • Ability to effectively coordinate multiple residents, priorities, and clinical needs.

  • Commitment to resident-centered care and interdisciplinary teamwork.

Preferred

  • Previous experience in care management, case management, skilled nursing, rehabilitation, or nursing leadership.

  • Experience with care planning and interdisciplinary clinical coordination.

  • Familiarity with MDS, PDPM, and skilled nursing documentation requirements.

  • Experience with PointClickCare or similar electronic health record systems.

  • Experience coordinating admissions, hospital returns, and transitions of care.

Why Join Us?

Care management is about more than reviewing a chart — it's about seeing the whole resident and making sure every member of the care team is moving in the same direction.

You'll have the opportunity to build meaningful relationships with residents and families while working alongside an interdisciplinary team focused on providing exceptional care.

We're looking for someone who takes ownership, communicates effectively, anticipates needs, and believes that great resident outcomes begin with great coordination.
