Salary Range: $37.00-$43.00 per hour base on experience 

Work Modality:  In-office, Monday through Friday from 8:30am-5:00pm with potential hybrid schedule and flexible hours.

If yes, here’s how you will contribute to the team:

As a Full-Time LVN Clinical Coordinator, you will acts as a liaison between patients, physicians, and clinical staff to ensure seamless, high-quality care for our home health patients from pre-admission through post-admission. You will play a pivotal role in ensuring patients and their families receive appropriate care coordination and utilization of services to ensure the highest quality care.

Your key responsibilities include:

  • Managing care for patient throughout the care continuum, including pre-admission, admission, and post-admission stages.
  • Coordinating services to ensure that patients' needs are met in a timely and effective manner.
  • Collaborating with healthcare providers, families, and interdisciplinary teams to develop and adjust care plans based on patient needs.
  • Monitoring the progress of patients and ensuring appropriate interventions are implemented when necessary.
  • Facilitating smooth transitions of care, ensuring all services and resources are in place for each patient.
  • Oversee patient care coordination, admissions and referrals, clinical documentation and review.
  • Order Management and administrative oversight.
  • Advocating for patients and families, ensuring their needs are met, and providing education as needed.

Must Have:

  • Successfully graduated from an approved and certified LVN program
  • Current and unencumbered license to practice as an LVN in the state
  • Ability to make quick judgments and work independently
  • Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

Nice to Have:

  • 2 years of experience as an LVN/LPN
  • Active patient or bedside care experience within the past three years

Compensation for roles varies depending on factors such as location, role, skill set, and level of experience. As required by state or local law, we provide a reasonable pay scale, including the hourly or salary range we reasonably expect to pay for this role.
