What are the responsibilities and job description for the Manager of Case Management position at American Advanced Management, Inc?
The Manager of Case Management works in conjunction with other members of the interdisciplinary team to coordinate and procure services and resources, as well as manage the care of patients, to achieve quality and cost efficient patient outcomes across the continuum of care. He/She applies review criteria to determine medical necessity for admission and continued stay, provides clinically based case management, discharge planning, and care coordination to facilitate the delivery of cost effective quality healthcare.
Qualifications:- Bachelors or Masters degree in nursing (RN) and minimum of 1-2 years’ experience as a Director or Manager of Case Management, OR
- RN License and Bachelors or Masters degree in social work from a program accredited by the council on Social Work Education OR
- RN License and Bachelors or Masters degree in a human service related field which includes: psychology, education, counseling, social services, sociology, philosophy, family and consumer sciences, criminal justice, rehab services, child development, substance abuse, gerontology, and vocational rehabilitation OR
- RN License and Bachelors in Liberal Arts or general studies with a concentration of at least sixteen hours in one of the fields listed in number three above
- Certification in Case Management, preferred
- Three years clinical experience, acute care setting preferred
- One year Utilization Review/Case Management experience
- Must be able to read, write, and speak English, as well as possess excellent verbal and written communication skills
- Basic computer skills required
