What You’ll Do as part of a team:
- Assess patient needs and develop personalized care plans after visiting them in the hospital, home or rehabilitation settings.
- Coordinate with healthcare providers, families, and community resources to ensure seamless care.
- Monitor patient progress and adjust plans as necessary to meet evolving needs.
- Advocate for patients, ensuring they receive the best possible care.
- Provide education and support to patients and their families regarding treatment plans and health conditions.
What We’re Looking For:
- Current RN License (in the state you’ll be working).
- Current, unrestricted driver's license and auto insurance
- Ability to travel within 100 miles of your home for patient appointments, hospital and rehabilitation visits
- Minimum of 2 years of trauma or ICU nursing experience, with some experience in case management.
- Ability to work independently and manage your schedule effectively.
- Comfort with computer skills and healthcare software data entry
- A highly organized individual with a focus on timeliness and attention to detail
