About the Role

We are seeking a compassionate, organized, and dedicated Case Manager to join our adolescent residential mental health treatment team. In this role, you will advocate for adolescents throughout their treatment journey by coordinating care, supporting treatment goals, facilitating communication between families and community providers, and helping prepare patients for a successful transition back to their home, school, and community.

The Case Manager serves as a key liaison between the treatment team, families, schools, and outside providers to ensure continuity of care and support each patient's academic, emotional, and behavioral success during and after treatment.

Key Responsibilities

  • Meet individually with adolescent patients to assess needs, build rapport, and support engagement in treatment.
  • Coordinate individualized case management services that address clinical, educational, social, and discharge planning needs.
  • Serve as the primary liaison between the treatment center and each patient's school to coordinate educational needs, obtain assignments and records, communicate treatment-related updates (when authorized), and facilitate a smooth transition back to the school environment.
  • Collaborate with parents, guardians, school personnel, therapists, physicians, and community providers to ensure continuity of care.
  • Assist patients and families in understanding treatment goals, discharge recommendations, and available community resources.
  • Develop and implement comprehensive discharge and aftercare plans in collaboration with the multidisciplinary treatment team.
  • Coordinate referrals and schedule follow-up appointments with outpatient providers, psychiatrists, therapists, and other community resources.
  • Participate in multidisciplinary treatment team meetings and contribute to treatment planning.
  • Maintain accurate, timely, and confidential documentation in Kipu.
  • Ensure all case management activities comply with HIPAA, DCF, AHCA, and organizational policies.
  • Advocate for patients and families by helping remove barriers to continued treatment, education, and community reintegration.
  • Provide ongoing communication and support to families throughout the patient's residential stay, as appropriate and authorized.

Qualifications

  • Bachelor’s degree in social work, Psychology, Health Care or related field preferred
  • Experience in mental health, substance use, or behavioral health case management, preferred
  • Strong communication, organizational, and coordination skills
  • Ability to work collaboratively within a multidisciplinary treatment team
  • Experience with Kipu, preferred
  • Ability to pass a Level 2 background check: HB531 | Florida Agency for Health Care Administration
