Key responsibilities
- Patient evaluation: Conduct comprehensive assessments of a patient's physical, cognitive, and emotional abilities and limitations within their home environment.
- Treatment planning: Develop and implement customized treatment plans with specific, measurable goals to restore function and increase independence.
- Therapeutic interventions: Provide hands-on therapy and instruction, focusing on activities of daily living (ADLs) such as dressing, bathing, and cooking.
Adaptive equipment and techniques: Introduce and teach patients how to use adaptive equipment (e.g., grab bars, specialized utensils) and compensatory strategies for tasks they struggle with.
Home safety modifications: Evaluate the home for safety hazards and recommend modifications to reduce fall risks and improve overall safety.
Patient and family education: Instruct patients and their caregivers on techniques, equipment use, and strategies to ensure progress continues after therapy sessions.
Care coordination: Collaborate with physicians, nurses, and other healthcare team members to ensure continuity of care and contribute to discharge planning.
Documentation: Maintain accurate and complete patient records, including progress notes and updated treatment plans.
