Job Title: RN Case Manager
Location: Pueblo, CO
Job Description:
We are seeking a compassionate and detail-oriented RN Case Manager to join our team. This role is responsible for coordinating patient care, conducting comprehensive assessments, and ensuring members receive appropriate, cost-effective services that support their health and wellness goals.
Schedule:
- Day Shift: 8:00 AM – 5:00 PM
- No on-call, weekends, or overtime required
- Travel required within the Pueblo, CO area
- Mileage reimbursement at $0.45/mile
Key Responsibilities:
- Perform comprehensive assessments for high-risk members.
- Develop and implement individualized care plans in collaboration with primary care providers.
- Ensure continuity of care by coordinating services and transitions between care settings.
- Identify appropriate and cost-effective levels of care while continuously reassessing member needs.
- Arrange non-medical support services such as housing, transportation, and other resources that support treatment compliance.
- Collaborate with physicians, social workers, discharge planners, and other healthcare professionals.
- Utilize specialty resources to achieve optimal patient outcomes.
- Maintain detailed documentation of clinical, functional, and financial outcomes.
- Promote health education, prevention, and wellness opportunities.
- Act as a patient advocate while upholding privacy and confidentiality standards.
Requirements:
- Associate or Bachelor’s Degree in Nursing.
- Current and unrestricted Colorado RN license or Multi-State License.
- Preferred experience with HCHB and hospice care.
- Strong ability to analyze complex medical information and make sound clinical decisions.
- Case Management certification preferred.
- Reliable transportation and active auto insurance required.
Pay: $50.00 per hour
Expected hours: 40.0 per week
