Position Purpose
The SA Specialist (SAS) provides community-based recovery services to participants receiving Community Support Team services. The SAS will be responsible for providing substance use treatment interventions in non-traditional settings, offer individual therapy targeting underlining factors to substance use, conducts substance use assessments for making recommendations, assist with conducting functional assessments, giving input into Person-Centered Plans, goal reviews, and discharging. Is an active member of the Community Support Team providing approximately 30 hours of direct care to adults with mental health and or substance use challenges.
- Essential Functions
Responsibilities:
- Demonstrates a working knowledge of the 12 Core Functions of Substance Use
- Use of evidence based/best practice modalities and treatment approaches such as Cognitive Behavioral Interventions, Illness Management & Recovery, Motivational Interviewing OARS, Harm Reduction etc.
- Facilitates community-based visits and treatment
- Assist with case management functions
- Presenting didactic/lecture psychoeducation
· Ensure and assist with On-call after-hour services to all Community Support Team participants
- Able to conduct/facilitate Substance Use Assessments and Clinical Updates for the purpose of treatment recommendations
- Able to develop a participant driven aftercare plan that transitions/discharges participants to least restrictive/higher level of care as needed
- Account accurately times sheets, vacation/leave request, and other reports required for program/department for statistical data and tracking outcomes
- Coordinate/collaborate with other providers/stakeholders involved in care of participant by facilitating minimum bi-weekly contacts. This may include attending court, treatment team meeting, submission of reports/written updates, and other required meetings
- Abiding by all HIPAA rules and regulations and all state laws outlined for substance use treatment; this means maintaining the highest and following the strictest laws of confidentiality state and federally
TECHNICAL SKILLS
· Basic computer skills for using Microsoft Word
· Ability to use Electronic Health Record system
· Writing skills that include sentence structure, grammar, and use of clinical terminology
ADMINISTRATIVE RESPONSIBILITIES
· Participate in weekly staff Community Support Team Meetings
· Attend collaborative meetings outside of the agency
· Submit progress notes within 24 to 48 hours of date of service in participant’s permanent record
· Ensure the minimal weekly service delivery of service hours for the overall team based on individual team members assigned service hours
· Ensure the submission of clinical documents and report(s) according to agency deadlines
· Submit all other assigned paperwork according to deadlines

Days/Work Hours
Primarily weekdays, eight hours per day, with flexibility to work in the evenings, weekends, or holidays if needed to accommodate/provide care to agency participants. Ability to cover shifts/weekends or as the need arises to provide First Responder services.
Travel
Community-based position which requires conducting home visits, attending collaborative partner meetings, accessing community resources, and approximately driving of 70 % based on participant need.
