Care Manager

We are looking for a skilled, detail-oriented nurse who thrives in a client-facing role and enjoys coordinating care that keeps seniors safe, supported, and thriving at home.

What You’ll Do

• Oversee and update personalized care plans
• Manage medication setups, refills, and safety checks
• Coordinate and schedule medical appointments
• Communicate regularly with families, providers, and caregivers
• Monitor changes in condition and intervene early
• Document accurately and maintain compliance standards
• Support caregivers with clinical guidance and client-specific coaching
• Advocate for the client’s best interest in all care decisions

Qualifications:

• Valid and active RN/LVN license in California.
• Proven experience in conducting thorough assessments and developing care plans.
• Knowledge of regulations and compliance standards in non-medical home care.
• Strong communication and interpersonal skills to effectively train and support caregivers if needed.
• Ability to work independently and as part of a collaborative team.
• Empathetic, patient-centered approach to care delivery.
• Excellent organizational and time-management abilities.
• Commitment to continuous learning and professional development.

Working Hours:

• Working hours may vary based on the needs of the client. This role may include evenings and weekends.
