The Client Care Coordinator (RN preferred) plays a key role in welcoming new clients into our agency, ensuring their care needs are fully understood, and supporting families through the decision-making process. As the clinical representative for Amada Senior Care, this RN conducts initial in-home assessments, develops individualized non-medical Plans of Care, explains service agreements, and helps families feel confident in moving forward with services.

This role is supported by a strong internal team—including our Care Support Coordinator, Hiring Manager, scheduling team, and interns who assist with call-offs—allowing the RN to focus on clinical oversight, assessments, documentation review, and family communication. The RN also provides ongoing clinical input when care concerns arise and ensures that caregivers are trained appropriately in collaboration with the Care Support Coordinator.

The RN may perform Private-Duty Nursing (PDN) on an as-needed basis, consistent with Iowa nursing regulations, under a separate skilled-services agreement. PDN documentation is limited exclusively to skilled tasks performed.

Qualifications

Current, unrestricted Iowa RN license.

Graduate of an accredited school of professional nursing.

Minimum one year of nursing experience; home care, geriatrics, or community health preferred.

Strong assessment, communication, teaching, and documentation skills.

Valid driver’s license and auto insurance meeting Iowa requirements.

Ability to complete home visits, including physical tasks as needed.

Able to pass all required background checks and screenings per Iowa Code.

Knowledge of infection control and HIPAA.

Primary Responsibilities

1. Initial Assessments & Intake

Conduct comprehensive in-home assessments to evaluate functional needs, safety risks, cognitive status, fall risk, and family support.

Review the Client Services Agreement with families; explain services, policies, expectations, and next steps.

Determine recommended care levels and caregiver hours.

Provide professional recommendations to support safety and independence in a non-medical setting.

Serve as the clinical liaison during intake and start-of-care, ensuring a smooth onboarding process.

2. Plan of Care & Case Management

Develop individualized, non-medical Plans of Care following Iowa Home Services regulations.

Update Plans of Care regularly based on client status, caregiver feedback, and family requests.

Communicate significant changes in condition to families, and when appropriate, healthcare providers.

Collaborate with community resources, hospice, rehab facilities, and healthcare partners.

Maintain concise, timely documentation per agency standards.

3. Caregiver Support & Supervision

Review caregiver care notes for clinical accuracy, safety concerns, and adherence to the Plan of Care; communicate updates or recommendations to the Care Support Coordinator as needed.

Follow up with clients and families regarding changes in condition, concerns, or satisfaction with services; provide clinical guidance and reassurance.

Collaborate with the Care Support Coordinator to ensure caregivers receive appropriate orientation, task-specific training, and competency validation.

Review and follow up on incident reports, including identifying contributing factors, determining if changes to the Plan of Care are needed, and recommending corrective actions.

Provide clinical insight and coaching to support the Care Support Coordinator and caregivers in delivering safe, effective care.

Occasionally conduct supervisory or spot-check visits when clinical issues arise, when requested by the Care Support Coordinator, or when a client’s needs change significantly.

4. Private-Duty Nursing (When Applicable)

(Performed under a separate PDN nursing service agreement, not under the home services license.)

Provide skilled nursing tasks within the RN’s Iowa scope of practice (e.g., medication administration, wound care, monitoring, disease-specific care).

Complete PDN documentation focused on skilled tasks performed and any notable clinical observations.

Communicate relevant updates to the Administrator and family.

5. Compliance & Agency Standards

Adhere to Iowa’s Home Services regulations and all Amada Senior Care policies.

Maintain confidentiality and follow HIPAA at all times.

Follow infection-control practices and safety procedures.

Participate in staff meetings, in-service education, and quality-assurance reviews.

Assist during regulatory reviews or audits, as needed.

6. Additional Responsibilities

Perform additional duties as assigned by the Administrator.

Pay: $66,500.00 - $74,000.00 per year
