As an MDS RN Coordinator, you will be responsible for conducting, collecting, and coordinating the development and completion of resident assessments in accordance with federal, state, and local laws, guidelines, and regulations. You will work closely with all levels of nursing personnel, consultants, physicians, and department managers to ensure that resident assessments are completed accurately and timely.
Responsibilities:
- Conduct and collect information, as well as coordinate the development and completion of the Minimum Data Set (MDS) in accordance with current rules, regulations, and guidelines that govern resident assessments.
- Assist in the implementation of care area triggers and resident care plans.
- Participate in facility surveys made by authorized government agencies.
- Work with the Interdisciplinary Team (IDT) to develop comprehensive resident assessments and care plans.
- Evaluate resident conditions and pertinent medical data to determine needs for special assessment activities or amendments to admission assessments.
- Contact and schedule participation by outside members of the care plan team, including the resident's representative and/or other interested family members.
- Participate in planning, conducting, and scheduling in-service training classes on assessment skills or techniques.
- Ensure accuracy and completeness in assessment functions and awareness of penalties for false certification.
- Coordinate the development of written plans of care for each resident, including measurable objectives and timetables.
- Ensure timely completion of resident assessments and care plan reviews.
- Communicate with residents and families to encourage participation in care planning.
- Professional:
- Current state licensure and a registered nurse (RN) degree
- Preferably a minimum of one year in a resident assessment position
- Progressive nursing experience with demonstrated leadership and teaching abilities
- Demonstrated skill in management principles of planning, organizing, and directing
- Personal:
- Strong communication skills, both oral and written
- Knowledge and awareness of the care needs of the elderly and/or chronically ill
- Knowledge and ability to teach adult learners
- Basic computer skills
- Ability to organize, analyze, investigate, and implement principles of infection control committee
- Must be able to work in a fast-paced environment with frequent interruptions
- Must be able to lift, push, and pull up to 50 pounds
- Must be able to stand for long periods and walk frequently
