Job Title/Position: Team Coordinator LVN/LPN
Reports To: Director of Patient Care Services and Team Coordinator Manager
Job Description:
A team coordinator LVN at a hospice agency serves as an essential link among patients,
their families, and the hospice care team. This role involves coordinating patient care by
scheduling appointments, managing medical records, and enhancing communication among
all parties involved. The coordinator organizes appointments for patients, physicians, and
hospice staff while also arranging necessary hospice care services. They manage supplies,
medication refills, and durable medical equipment for all patients in collaboration with the
team. Additionally, they assist with the admission and discharge processes, ensuring
smooth transitions. Effective communication with the hospice team comprising physicians,
nurses, social workers, chaplains, and volunteers is vital, as is facilitating dialogue
between the hospice team and patients or their families. The coordinator also plays a key
role in developing and implementing patients' care plans and participates in team meetings
and case conferences. They ensure the accuracy and compliance of interdisciplinary team
agendas by facilitating weekly meetings.
Essential Job Functions/Responsibilities:
- Scheduling appointments for patients, physicians, and hospice staff members.
Coordinating hospice care services as needed. - Coordinates supplies, medication refills, and DME as needed for all patients with the
team. - Assisting with the admission and discharge processes for patients
- Communicating effectively with the hospice team, including physicians, nurses, social
workers, chaplains, and volunteers. - Facilitating communication between the hospice team and the patient/family.
- Assisting with the development and implementation of the patient's plan of care.
- Participating in teams meetings and case conferences. Coordinator and facilitate
interdisciplinary team agendas and meeting weekly ensuring accuracy and compliance.
Position Qualifications
- Licensed Vocational Nurse/Licensed Practical Nurse (LVN/LPN) with current state
licensure. - Experience in hospice is highly desirable.
- Excellent organizational and time-management abilities
- Effective communication and conflict resolution skills
