Position Summary
We are seeking a Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) to join our hospice care team in Palo Alto. This role plays a vital part in ensuring patients receive personal, supportive care in the comfort of their home or care facility.
Key Responsibilities

Deliver individualized personal care such as bathing, grooming, and dressing

Assist patients with daily living activities and comfort measures

Monitor and report changes in patient condition to the care team

Provide basic healthcare support such as medication reminders and non-sterile bandage changes

Offer emotional support and companionship to patients and families

Educate family members on how to safely support their loved ones
Qualifications

Certified Nursing Assistant (CNA) – Required

Certified Home Health Aide (CHHA) – Required

Previous experience in hospice, palliative care, or home health – Preferred
